服務目標「生活再動計劃：賽馬會居家安老新里程」為體弱的長者建立嶄新的家居復康服務，這個新模式結合科技、醫護團隊和護老者的參與，為長者重整健康生活模式，以及提供康復訓練。服務內容透過上門探訪活動，教導長者使用平板電腦，支援長者使用「生活再動流動應用程式」。
