兒童脊科基金（CCF）是非政府資助的小型社福機構，將於2025年6月14日(六) 舉行2025全港賣旗日。CCF是全港唯一關注兒童脊骨神經健康的慈善團體（編號：91/8705），為香港社會服務聯會及香港公益金的會員機構，賣旗日善款將用於：兒童脊科義診計劃、 優惠脊科服務及免費到校兒童脊骨檢查，同時擴大為兒童提供脊骨檢查及調整服務的規模。誠邀各位報名成為街頭賣旗日義工，支持我們照顧更多有需要孩子的脊骨神經健康！❤️🙏🏽

- ***重要，請細閱本備註中資訊*** 日期︰2025 年 6 月 14 日 (星期六) 時間︰上午 7:00 – 中午 12:00（可因應閣下需要調節時間） 地點︰全港各區 凡參與街頭賣旗義工各均可獲得優惠脊骨檢查券及獲義工參與證書 注意︰根據社署要求，14歲以下兒童必須由成年人一對一陪同 個人資料收集聲明 閣下的姓名、身份證號碼、聯絡方式等個人資料，用以處理 閣下申請參與是次賣旗義工服務的安排，包括領取旗袋時核實身份、發出參與證明及安排鳴謝，請給予英文全名(須與身份證明文件相符)及身份證號碼(英文字母數字之首 4 個字元)，個人資料的收集只供是次申請成為賣旗義工用途，資料亦將於完成賣旗活動後銷毀。「*」必須填上中、英文姓名及聯絡電話，如閣下未能提供，本會可能未能處理閣下的申請及服務安排。資料只會供處理申請的人員參閱，作為審批、聯絡、服務申請和編排的用途。除非法律許可或有所規定，本會不會在未得到你的同意下透露你的個人資料予第三者。根據《個人資料(私隱)》條例，本會及轄下中心確保妥善的儲存及處理個人資料，本會可能使用你的個人資料，以便本會日後與你通訊、處理報名、發出收據、研究、分析、統計、籌款、收集意見、活動或訓練課程之邀請用途，及與本會相關之事宜。如果你對個人資料的使用有任何的查詢，請電郵本會(info@ccf.hk)，謝謝！ 報名本活動將表示你同意提供你的個人資料以供是次參與賣旗義工之用及同意兒童脊科基金使用本人的個人資料於擬作出的籌款、活動或訓練課程之邀請用途。
