Description
本港獨居和雙老長者人口持續增加，當中不少基層長者與社會疏離，缺乏支援網絡。因此，香港賽馬會慈善信託基金撥款超過一億八千八百萬港元，在二零二三年於六個地區推行為期四年的「賽馬會『樂載耆蹤』社區長者支援計劃」，並在二零二四年十一月開展第二期，將計劃拓展到其餘十二區推行爲期三年的計劃。計劃旨在透過社福機構及地區組織，以多元化的方式，於區內積極發掘有服務需要長者，配合香港大學建立的數據平台，作出即時支援或服務轉介，協助他們重新與社區連結。計劃亦為有需要長者提供家居維修及改善服務，以提升安全，達至居家安老。
