「生活再動計劃：賽馬會居家安老新里程」由香港賽馬會慈善信託基金捐助，聖雅各福群會協辦，為體弱的長者建立嶄新的家居復康服務，這個新模式結合科技、醫護團隊和護老者的參與，為長者重整健康生活模式，以及提供康復訓練。
