一起為認知障礙症長者創造更好的生活!
由香港聖公會黃大仙長者綜合服務中心舉辦的「智友醫社同行計劃」， 以「醫社合作」模式，加強社區層面上的護理服務 元素，為患有輕度或中度認知障礙症的長者及其護老者在社區內提供適切的支援服務。
計劃為參加者於認知能力、身體機能、心理社交及護老者支援提供訓練，並透過小組，讓長者在愉快及無挫敗的環境中學習，以鼓勵持續參與。

- 日期: 暫無日期限制，持續需要義工 時間: 1) 每星期一節( 3.5 小時)，如能固定每星期同一時間當值更佳( 例: 每星期四之早上)，並需要要持續參與3個月或以上 2)早上 9:15am-12:45pm 工作性質包括： 1) 協助長者參與活動 2) 活動進行時看顧長者 3) 協助接送長者由家居往返活動地點(按需要) 4) 有需要時協助戶外活動進行
